Your placement with the Practice will last for six months and we look forward to welcoming you to the team.
Personal Documentation
It is essential that you provide the Practice with the following personal documents:
Prior to employment by the Practice:
WYCSA forms
You will be required to complete a joining form and grant form to submit to West Yorkshire Central Services Agency.
GMC Certificate & Birth Certificate or Passport
The Health Authority requires sight of the original documents prior to commencement in order to approve your employment by the Practice. They will be returned to you after processing and the necessary copies have been taken for our records.
You may wish to deliver the forms personally to WYCSA at Brunswick Court, Bridge Street, Leeds, LS2 7RJ. Their telephone number is 0113 295 2500.
From Woodhouse Lane proceed past Morrisons on your left at the Merrion Centre heading for the traffice lights at the junction of Albion Street, Great George Street and Merrion Street. Turn left into Merrion Street and continue to the junction with New Briggate (The Wren pub is on your left and The Grand Theatre over to your right). Passing through the traffic lights, go straight across down the narrow street to join Vicar Lane. At this point, Templar Street is the road opposite you and this leads down to Bridge Street. Proceed past Gower Street and Brunswick Court is a new brick-built complex on your right. Alternative routes can be found at Multimap.co.uk.
Medical Defence Insurance
Confirmation of your current cover in the form of an up-to-date certificate is also required by the Health Authority prior to commencement of your employment with the Practice. This too will be returned to you following processing of the information.
Please obtain and produce a breakdown letter to submit for reimbursement of fees.
On commencement of employment by the Practice:
Tax Forms
In order to ensure that tax is deducted from your salary at the correct rate, we require the P45 form from your previous employer. This should be handed to Mrs Wendy Lawson, the Business Partner, or to Mrs Maureen Naylor, the Finance Manager, by the end of the second week in post to enable the appropriate deductions to be made with effect from your first month’s salary. If the P45 is received later, the salary paid will reflect the tax adjustments that will have to be made in the absence of the form.
Car Insurance
Under your contract you are required to:
- Provide and maintain a motor car or other suitable transport to enable you efficiently to carry out your responsibilities under your contract
- Maintain an effective current driving licence
- Conform to the statutory requirements relating to motor vehicle insurance.
Please note: It is essential that you inform your insurance company that you intend using your motor vehicle for business purposes to ensure that your insurance cover is adequate.
Practice Equipment
Whilst it is expected that you come prepared with the necessary personal equipment, i.e. Doctor’s bag, stethoscope, etc., the Practice may be able to make up for any other deficiency on a short-term basis from a pool of items. It is a condition of your Contract that anything borrowed from the Practice is returned in good condition at the end of the period, prior to your leaving.
Timetables
All working timetables are compiled at least one month in advance by the Group General Manager, Mrs Jill Peel. Please give as much notice as possible for any annual leave/study leave.
Consultations
You will be expected to attend at all three sites – Windsor House Surgery in Morley, Shenstone House Surgery in Churwell and Adwalton House Surgery in Drighlington.
Initially, the duration of your consultations with patients will be 15 or 20 minutes. Over time, this will be adjusted downwards to conform to the standard appointment time of 10 minutes. No changes will be made without your agreement following discussion with your Principal.
Throughout your General Practice training, please feel free to refer to your training Principal or other GP Partner regarding any medical problem or clinical treatment requiring clarification.
Home Visits
Home visits are allocated by the Surgery Manager, Ms Liz Field. Your list will be available towards the end of morning surgery.
On-call arrangements
You will be expected to take your turn/assist with on-call duties.
Continuing Education
Group tutorials are held at Adwalton House Surgery every Thursday morning between 8.30 am and 11.00 am in one of the Training Suites.
They are conducted by the Partners, clinical sessions being taken by the training Principals and those with a business orientation by Mrs Wendy Lawson, Business Partner. Your input in respect of the subjects for discussion is welcomed.
The details of individual tutorials will be available during your stay with the Practice.
The dedicated medical library adjacent to the Training Suites provides reference material, internet links and TV and video facilities.
If you wish to borrow books, please see Margaret Armstrong, who will book them in and out for you. Remember to return all borrowed books before you leave at the end of your placement.
